To all parents of students entering grades Pre K or K, 1, 3, 5, 7, 9, and 11 in September 2020:

The New York State Education Department requires physical examinations by a physician, nurse practitioner or physician’s assistant for students in certain grades. Beginning in September 2018 those grades are changing to grades Pre K or K, 1, 3, 5, 7, 9, and 11.

This means there will be some students who will require exams two years in a row if they had a physical required this year. This is not being imposed on you by the district. It is a new mandate by the State Education Department, and the district must follow the new regulations.

When you see your physician or other health care provider for a routine annual physical exam this year, please ask for a health certificate to give your current school nurse. This is also a good time to schedule a full dental screening for your student and to bring that dental certificate in as well. It can take weeks to schedule a physical exam. Please plan ahead.

• All physical examinations are valid for 12 months prior to the first day of school and 12 months prior to the first day of the month of each sports season- fall, winter, spring. Sports exams are good through the last day of the month in which they were conducted.
• Delays may result in your student’s inability to be assigned physical education classes which could affect their final grade, participation in extracurricular activities, including recess or playground, and other club, intramural, or interscholastic activities.
• Certificates may be hand-delivered, mailed, or faxed to the school nurse at the address/phone listed below
